Program Requirements
Patients of Downriver Community Services are eligible to apply for our Sliding Fee Program. This program is funded in part through federal grant monies and offers discounted fees to qualifying patients. Preventive health care should be affordable to everyone regardless of income.
Residency Requirements

- St. Clair County (The Entire County of St. Clair)
- Macomb County (Within Lenox Township, Richmond, Richmond Township, Chesterfield, New Baltimore, Ray Township, and the Village of New Haven)
The following documentation of all that applies to your household is required to determine eligibility:
- Last 2 pay stubs of all working household members
- Proof of any other household income (child support, worker’s comp, unemployment, rental income, pension, etc.)
- Social Security New Benefits Income Statement or other proof of current S.S. income (if applicable)
- Most recent Federal Income Tax return
- Birth certificates of all children 18 and under to be covered by the program
- Birth Certificates for all women 19 – 44 (Plan First application required, unless already enrolled)
- Valid Driver’s license, State of Michigan ID card or other proof of current residency
- Social Security Card of all applicants
- Medicaid or Medicare card (if applicable)
Covered Services
Services performed at our facilities and prescriptions written by our physicians will be discounted under the terms of this program. Discounts do NOT apply to charges imposed by hospitals, laboratories, or other physicians. The program will cover select lab tests when performed by a physician at our facilities. A list has been included with your handbook. All other lab tests are the responsibility of the patient. If you have questions about the cost of lab tests not covered by the program, please discuss them with your doctor or the laboratory technician. Our nursing staff may be able to contact the lab to obtain a price for you. In many cases the lab will discount their fees for our patients.
Income Verification
Although we use current income to establish eligibility, we must have copies of your most recent tax return. If you do not have a signed copy, you may call (800) 829-1040 to request one.
Medicaid and MI Child
In the event you claim no income or you qualify at the 0% discount rate you will be required to apply for the State Medicaid Program (exception: seniors with Medicare). Also, your children may qualify for the state insurance program called MI Child. Please inquire with our staff. They can assist you on how to apply or call your local Department of Human Services, St. Clair County DHS, (810) 984-7500, or Macomb County DHS, (586) 469-7700.
If you are denied Medicaid you must provide us with a copy of your denial notice from DHS in order to remain eligible for the Sliding Fee Program. Denial due to failure on your part to complete the Medicaid application or provide additional documentation will result in you becoming ineligible for our program.
