Homeless Health Care Project
Downriver Community Services Homeless Health Care Project has either directly provided or coordinated the provision of health and support services to the homeless population since its incorporation in 1988.

Presently, the program receives Federal Funds through a Section 330, US Department of Health and Human Services (HHS), Health Resources and Services Administration (HRSA), as well as support from the Bureau of Primary Health Care Services (BPHC) Grant. In the mid 1990's, the grant was combined with the Community Health Center (CHC) Grant.

The mission of Homeless Health Care Project is to ensure that men, women, and children in the St. Clair County and Macomb County area who are homeless will receive the quality health care and supportive services they need in order to live at the highest possible level of self-sufficiency. The Homeless Health Care Project strives to achieve this mission through numerous activities that include advocacy, outreach, prevention, public awareness, and collaborative relationships with other providers.

Guidelines: Eligibility to enroll in the Homeless Health Care Project are as follows...
  • They must qualify as homeless
  • Must be able to receive health care services in Macomb or St. Clair Counties
  • No income requirements

The Downriver Homeless Health Care staff members are trained professionals dedicated to providing high quality medical and support services to the homeless population in both Macomb and St. Clair counties. The staff includes: county case managers, licensed counselor(s), medical assistants, registered nurses, nurse practitioners, physicians, and support staff.
Services Provided
Clients must meet with Homeless Project Staff to determine eligibility.
