Homeless Services Provided

  • Primary Health Care
    Clients must meet with Homeless Project Staff to determine eligibility. If a person meets program requirements, they will be enrolled into the Homeless Health Care Project. Primary medical care is available to the homeless at various sites and shelters throughout Macomb and St. Clair counties. Medical services and enrollment is also offered at our New Haven Clinic and Algonac Medical Clinic.

  • Prescription Assistance
    Clients are able to receive medications prescribed by Nurse Practitioners and physicians. Narcotics, controlled substances, and psychotropic medications are not provided or paid for by the program.
  • Wound Care
  • Flu Vaccines - When Available
  • TB Testing
  • Substance Abuse & Mental Health Counseling
    Counseling services are available by licensed professional staff.
  • Health Education Services
  • Case Management Services
    Case managers are members of the Health Care Team. They are available at both Algonac and New Haven facilities to complete enrollments into the program and make appropriate referrals.
  • Emergency Dental & Vision Services (Dependent on funding availability)
    A patient must be enrolled in the Homeless Health Care Project to qualify for emergency dental and vision services. Approval for services will be determined by medical providers. Appointments will be scheduled and coordinated by the case managers.
