Downriver Community Services offers 3 full time case managers to assist our patients with services beyond the medical office.
- Access to identifying local agencies that assist with food, rent, clothing and utilities
- Referrals for Mammograms and Pap Smears at low cost or no cost
- Referrals for Low or no Cost MRI, low cost CT Scan
- Assistance with navigating Medicaid/Medicare
- Application Assistance for Plan First, Healthy Kids and MIChild
- Access to State of Emergency Relief Application to assist patients with evictions, utility shut offs, back taxes and possible down payments for housing
- Referrals for substance Abuse Counseling/Inpatient Treatment
- Access to Community Mental Health for Outpatient Services
- Referral to local Tax Preparation Services at low or no cost if available
- Support Groups in the area
- Complete resource library for information on Parenting, Anger Management, Drugs, Depression, Age Development, Domestic Violence Information and Much, Much More...
